But if you need to transition to assisted living, memory care or skilled nursing, the Navigator will help you every step of the way. At your request, the Navigator can also help schedule medical-related appointments and coordinate transfers and paperwork for hospital procedures. When you return to Sagewood from the hospital, the Navigator will ensure the health services you need are all in place to support a quick recovery and a return to the life you love.
